A. Certificate of Insurance evidencing Commercial General Liability Insurance

A copy of your Schedule of Forms and/or Endorsements
No Action Over exclusion
No Exclusions for Damage to Work Performed on your behalf.
No Bodily Injury to Independent Contractor Exclusions
No Residential exclusions for custom or single-family homes
No Subsidence exclusions (if applicable)
Providing coverage on a full occurrence basis.
Covering premise and completed operations.
30-day Notice of cancellation
Primary and Non-Contributory Coverage
Such insurance shall contain all standard comprehensive or commercial General liability broad form terms and conditions including coverage for:
1. Premises Operations Liability for bodily injury and property damage
2. Personal Injury Liability (without any employee exclusion endorsement, products),
3. Broad Form Blanket Contractual liability,
4. XCU coverage (required of subcontractors involved in utility work, excavation, mechanical, electrical and/or plumbing work), and
5. Products/Completed Operations coverage.
With limits of not less than
1. $1,000,000 Each Occurrence
2. $2,000,000 General Aggregate
3. $2,000,000 Products and Completed Operations Aggregate
With Additional Insured endorsements for Ongoing and Completed Operations in favor of:
1. ARCA Builders
2. Owner
With Waiver of Subrogation issued to above Additional Insureds

B. Certificate of Insurance evidencing Auto Liability Insurance

With limits of not less than:

$1,000,000 Combined single limit for Bodily Injury and Property Damage For Any Auto, or for all owned or scheduled, non-owned, leased and hiredautomobiles, trucks, trailers, and semi-trailers, including but not limited to any machinery or apparatus attached thereto.

C. Certificate of Insurance evidencing Worker’s Compensation and Employer’s Liability Insurance as required by applicable law or regulation. Employer’s Liability Insurance shall be provided.

With limits of not less than
1. $1,000,000 Each accident for bodily injury by accident
2. $1,000,000 Policy limit for bodily injury by disease
3. $1,000,000 Each employee for bodily injury by disease
With Waiver of Subrogation Endorsement

IMPORTANT NOTICE: All of the above insurance shall be maintained in force from the commencement of the Work until final completion of the Project and through Subcontractor’s contractually agreed upon warranty period obligation, whichever comes later. In addition, Commercial General Liability Insurance coverage, including additional insured coverage for Owner and Contractor, shall be maintained in force until expiration of the applicable statute of limitations for claims related to latent defects and construction improvements for real estate.
